ph3FLSA: Exempt /h3 h3Reports To: Clinical AD /h3 h3Physical Requirements: The ability to transfer people with disabilities using approved techniques bending, walking, standing, lifting 50lbs, kneeling is also required. /h3 h3Hours of Work: Monday thru Friday, 8:30am to 4:30pm. with flexible schedule to allow for time spent in residences early evening and early morning. Fulltime 37.5 hours per week (Weekend, on-call or holiday work as needed) /h3 h3Job Summary: Provide physical therapy assessments and treatment to ensure clinical and regulatory compliance. Schedule and provide therapy in the Article 16 clinic, as well as in Day and residential programs as needed. /h3 h3Job Qualifications /h3 ul liIndividual must be licensed and currently registered as a Physical Therapist by the NY State Department of Education. /li liSpecialized training or one year of professional experience in treating developmentally disabled preferred /li liValid NY State driver's license required. Some travel required. /li /ul h3Major Responsibilities /h3 ul liProvide Physical Therapy services to individuals in programs, in compliance with NYS, Federal, agency and department regulations. /li liProvide training to agency personnel to include in home and work sites. /li liRecord and maintain individual's PT record in compliance with all state, federal, local regulations and agency policy. /li liParticipate in team meetings and surveys as part of individuals overall plan of care. /li liCommunication that is appropriate, accurate and timely. /li liPursue personal/professional development and attend mandated agency training as required. /li liMonitor all adaptive equipment and schedule repairs or replacement as needed for wheelchairs, walkers, and orthotics. /li /ul h3Disclaimer – /h3 pDisclaimer – The above statements are intended to describe the general nature and level of work being performed by people assigned to this classification.
